Overview
A scar may cause discomfort because of its colour, width, raised or depressed surface, or pulling on surrounding tissue. Scar revision is planned around the features that matter most. Timing is important because new scars change as they mature; a contracture that limits movement may have a different priority. Surgical revision aims to replace the old scar with a new scar in a more suitable position or shape. Non-surgical methods may be considered for colour and surface irregularities. Skin characteristics, previous healing and tension in the area affect the result. The aim is a realistic improvement in appearance or function.
What is evaluated?
- The age, cause, width and ongoing changes of the scar are recorded.
- Examination of raised or depressed areas, color differences and tethering to surrounding tissue.
- Pain, itching or joint movement limitation are evaluated.
Approach and options
- For new or selected scars, follow-up, silicone and appropriate surface care may be considered.
- In cases of redness or tissue irregularity, laser and other local treatments may be considered.
- Surgical revision and tissue rearrangement may be planned in case of width, malorientation or contracture.
Recovery and follow-up
If surgery has been performed, the new incision is protected from tension and friction. During follow-up visits, wound closure, scar thickness and color change are evaluated. Rehabilitation may be required for scars that affect movement. The final appearance is elusive early on; During scar maturation, the care plan may change and the need for additional treatment will be reconsidered.

- The scar widens or swells again
- Infection, bleeding and wound dehiscence
- Pigment or sensory change
- Inadequate improvement and need for revision
